Facility Evaluation Report
Licensing Program Analyst (LPA) Avelina Martinez arrived at this facility unannounced on December 05, 2025, at 9:45 AM to conduct a Health and safety inspection. LPA Martinez met with Administrator, Aaron Fintel, and explained the purpose of the visit.
The purpose of the visit today is in response to a quarterly health and safety inspection. LPA Martinez conducted a walk through of the facility and evaluated the physical plant to ensure the health and safety of the residents in care. LPA Martinez toured the facility with Aaron Fintel on December 05, 2025, at 10:00 AM. Areas inspected are including but not limited to the kitchen, resident bedrooms; resident bathrooms, living and dining room and outdoor areas. LPA Martinez observed the facility to be free of odor, clean and in good repair. LPA Martinez observed that all rooms are equipped with the required furniture and sufficient lighting throughout the facility.
LPA Martinez reviewed resident files, staff files, medication administration records (MAR). LPA Martinez reviewed four resident files, and there were no deficiencies noted. LPA Martinez reviewed four staff files, and there were no deficiencies noted. LPA Martinez reviewed four medication administration records, and no deficiencies were noted. Staff training is ongoing and new a EMAR system has been implemented
An exit interview was conducted, and a copy of this report was provided to the facility.
