Facility Evaluation Report
Licensing Program Analyst (LPA) Christina Valerio arrived unannounced to conduct an annual required inspection. LPA Valerio met with Administrator Hiyasmin Samala, and explained the purpose of the visit.
LPA Valerio and Administrator toured the facility to ensure compliance with Title 22 regulations. LPA Valerio observed three residents sitting in the living room engaging in morning activities. LPA Valerio observed the staff area, living room, kitchen, dinning room, resident bedrooms, and resident bathrooms. The facility was observed to have an adequate food supply and an emergency supply of food. The temperature inside the home was within the regulatory range. Resident bathrooms were observed to be fully stocked with hygiene supplies, clean, and free from debris. Resident bedrooms were observed to be fully furnished, personalized to each individual, and free from obstructions. Medications, sharps, and toxins were observed to be locked and inaccessible to residents in care. The fire extinguisher was last serviced on 08/23/2025. The last fire drill was held on 09/13 and 09/14/2025. Staff were observed assisting residents with breakfast, snacks, and daily morning activities.
LPA Valerio reviewed three (3) staff files and two (2) resident files. All files were up to date and had required documentation. LPA Valerio requested the following documentation be sent to LPA Viarella: LIC 500, LIC 308, LIC 610, and copy of the surety bond
Per California Code of Regulations (CCR) - Title 22, no deficiencies were observed today. An exit interview was held, and a copy of the report was provided.
