Facility Evaluation Report
On 3/28/25 at 12:40pm, Licensing Program Analyst (LPA) Kevin Gould conducted an unannounced case management inspection to provide guidance of repairs required to ensure the facility remains in compliance with title 22 regulations and to complete the change of ownership process.
LPA observed the window screens/sliding screen door in all resident bedrooms and one resident bathroom are in need of repair/replacement. LPA also observed the baseboards below the shower in one resident's bedroom is in need of replacement as the baseboard has become waterlogged and is decomposing. LPA also observed the shower door was recently broken and is in need of replacement/repair.
LPA advises that all items are required to be repaired to ensure the facility remains in compliance with title 22 regulations and the department can complete the change of ownership process.
Exit interview conducted and a copy of this report and advisory notes were provided to the facility.
