Facility Evaluation Report
Licensing Program Analyst (LPA) Avelina Martinez arrived at this facility unannounced on February 04, 2026, at 12:45 PM to conduct a case management visit. LPA Martinez met with Steve Bush and explained the purpose of the visit.
The purpose of the visit today is to discuss the following deficiency: Incidental and Medical.
During record reviews, it was learned that facility staff did not assist resident 1 (R1) with their prescribed medications as needed. On November 20, 2025, R1 was prescribed a permethrin topical cream. The facility staff did not assist with acquiring the medication. As a result, the resident did not receive the required medication treatment to treat lice. The facility will be cited 87465 Incidental Medical and Dental care. The deficiency can be found on the 809-D page.
An exit interview was conducted, and copy of this 809 report, 809-D page, and appeals rights were given to the facility at the end of this visit.
