Facility Evaluation Report

Licensing Program Analyst (LPA) Kevin Mknelly arrived at the facility unannounced on 6/2/26 to conduct a Annual Inspection utilizing the CARE inspection tool guidelines. LPA met with manager, Kimberly Sena who assisted with the visit.

This program shares a campus with two other day programs operated by the same Licensee. This campus also houses several other programs. LPA conducted all three annuals at the same time.

There are 24 clients present with 9 care staff.

LPA toured the interior and exterior of the facility together with staff to ensure health and safety of residents in care. Areas toured include but are not limited to: classrooms, bathroom, kitchenettes, rest areas, laundry areas, and walkways. In the areas toured no immediate health, safety, or personal rights violations were observed. The program is very clean and clients in care appeared to have their needs met with adequate staffing.

LPA advised fees still due by the license anniversary date.

No deficiencies are being cited as a result of todays inspection.
