The inspector’s account

Licensing Program Analyst (LPA)Cynthia Tamayo made an unannounced visit to this facility to conduct an annual required inspection on 07/13/2023 at 9:00 AM. LPA Tamayo met with Pratibha (Nina) Patel (S2)and explained the purpose of the visit. The current census is 25 with 5 facility staff.

The LPA toured the facility with S2. The facility is licensed for 120 ambulatory consumers and 20 non-ambulatory clients ages 18 and over.

LPA and inspected the the class room, storage rooms, staff break room, offices, bathrooms, and outdoor areas. Furniture and furnishings were enough to meet the needs of residents. The facility temperature was within the required range of 68 and 85 degrees. The facility's water temperature measured 109.5 degrees Fahrenheit, which is within the required range of 105 and 120 degrees. The facility is sanitary and clean. The day program is not dispensing medication at this time. The first aid was inspected and it was complete.

The facility exterior passageways were clean and clear of any obstructions. There is a patio area for client use, picnic tables and seating were observed to be in good condition. Clients are supervised at all times when they are outside. LPA observed two locked shed which contains general gardening equipment. The last emergency drill was on 8/12 2025 in which alternating exits are being used during drills.

LPA reviewed 8 client files, and all files were up to date. LPA reviewed 3 staff files, and files were up to date. LPA reviewed staff criminal record clearances and a review of staff records indicates that all facility staff or other individuals who require caregiver background checks are fingerprint cleared and associated to the facility.

The facility is in compliance with California Code of Regulations, Title 22 and Health and Safety Code, there were no deficiencies cited at this time. An exit interview was held, and a copy of this report was provided to the facility.
