Facility Evaluation Report
Licensing Program Analyst (LPA) Cassie Yang arrived unannounced at the facility to conducted an annual inspection utilizing the care tool. LPA met with Workforce Inclusion Manager, Mary Grimes, and explained the purpose of the visit.
Today's census during LPA's visit was 0. LPA arrived after day program has ended.
LPA and Workforce Inclusion Manager conducted a tour of the facility to ensure the health and safety of participants in care. LPA observed staff to be cleaning up before closure. LPA observed kitchen, restrooms and the common areas to be clean. LPA was informed today's attendance was 42 participants. Workforce Inclusion Manager explained that the program conducts three separate sessions of 8-10:30; 11:30-2 and all day. Workforce Inclusion Manager further explained that the program implements facility activities along with community based activities to prevent fire clearance violations.
Facility was observed to be clean, sanitary and in good repair. In areas toured, no immediate health, safety and personal rights violation observed.
CARE tool completed and no deficiencies cited.
Exit interview conducted and a copy of report was provided.
