LPA also observed hygiene supplies for residents in care including soap, shampoo, cream, lotion and perfume. Also, during the facility observation, Resident2 (R2) was present,and playing video games in their bedroom. Per observation, LPA observed R2’s clothes to be clean and observed no visible stains. LPA also observed R2 to be well-groomed, clean shaven, and free from odor. LPA conducted record review of Resident1 (R1) including their Individual Program Plan (IPP) report, Individual Services Program (ISP) report, care notes for November 2023 and medication.
An additional record review of R1 was conducted at R1’s day program which includes their ISP report and care notes. Per review of the day program care notes dated 2/1/24 to 3/27/24 revealed that on most days, staff noted R1 to be cleaned in appearance. Further record review revealed that R1 has an ongoing behavior of refusing to shower. Per interview with facility staff and day program staff revealed that R1 would go to day program without showering. Per interview with facility staff revealed that resident who refuses shower would be assisted with a wipe down method using wet cloth similar to a sponge bath. Additionally, during a visit at R1’s day program on 3/28/24, LPA observed R1 to be clean in appearance.
Based on the information gathered throughout this investigation, there is not a preponderance of evidence to conclude that staff did not provide clean clothing to resident(s)., therefore, this allegation is UNSUBSTANTIATED. Note that an unsubstantiated finding means although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur.
