The inspector’s account

On 01/29/2026, Licensing Program Analyst (LPA) Pang Lee conducted an unannounced annual inspection at the facility. Upon arrival, LPA Lee met with Administrator Naomi Guevarra and explained the purpose of the visit. The facility’s current census was four (4) clients with two (2) staff present.

LPA Lee conducted a physical plant inspection, including but not limited to the common areas, kitchen, dining area, client bedrooms, client bathrooms, laundry room, garage, staff room, and outdoor courtyards, to assess compliance with Title 22 regulations. The facility was observed to be clean, free of odors, and in good repair. Client bedrooms were properly furnished and equipped with appropriate bedding and lighting. No bodies of water were present on the premises. The facility has sufficient seven-day nonperishable and two-day perishable food supplies. Hot water temperature in the client bathroom sink measured 115.7 degrees Fahrenheit. Smoke and carbon monoxide detectors were observed to be in compliance with fire safety requirements. The fire extinguisher was located in the kitchen and was last serviced on 10/09/2025. The most recent fire drill was conducted on 12/11/2025. LPA Lee observed that the facility has a public telephone located in the dining area and that all required postings were properly displayed. The facility thermostat was observed to be set at 70 degrees Fahrenheit. Toxins were observed to be stored in the laundry cabinet and were locked and inaccessible to clients. Sharp knives were observed to be locked in the staff room cabinet and inaccessible to clients. Medications were reviewed and found to be stored securely, locked, and inaccessible to clients. The first aid kit was inspected and contained all required components.
