Facility Evaluation Report
On February 15 2023, at 8am, Licensing Program Analyst (LPA) De Anna Williams-Lyons made an unannounced visit to conduct facilities required annual inspection. LPA met with Sararatu Okolo, Administrator, and informed her the reason for the visit.
Prior to initiating today's inspection, LPA completed required COVID-19 Department protocols, wore a surgical mask and was screened per Covid-19 precautionary measures upon entering the facility.
The Administrator's Certificate is valid expiring 6/20/23. The facility's temperature was 69 degrees.
LPA inspected the interior and the exterior of the facility including the common living spaces, resident bedrooms and bathrooms, and kitchen. Bathrooms and bedrooms were clean and in good repair. Food supply is adequate for 2-day perishable and 7-day nonperishable. Hot water temperatures were taken and measured at 105 degrees F, which is within the allowed range of 105-120 degrees. Smoke alarms are in good working order. Washer and dryer was present and operate properly. Toxic substances, laundry and cleaning supplies are inaccessible. LPA found the first aid kit to be complete. LPA inspected the exterior grounds of this facility. There are no bodies of water on the premises. Passageways are free of obstruction and potential hazards.
Sararatu and LPA completed the inspection tool questionnaire with no issues or advisories to report.
As a result of this visit, no deficiencies were cited, per Title 22 Regulations, Division 6.
The administrator shall submit updated copies of the LIC 500 Personnel Report, LIC 308 Designation of Administrative Responsibility, LIC 610D the Emergency Disaster Plan, and copy of current Liability Insurance to update the facility file in our Regional Office. Administrator shall submit the listed documents to Licensing later than March 15, 2023.
