Facility Evaluation Report
Licensing Program Analysts (LPAs) Holly Williams and Charlie Yang arrived announced to conduct a pre-licensing inspection on 3/27/25. LPA Williams met with applicant Rosa Dinh and explained the purpose of the visit.
LPAs interviewed Applicant Rosa Dinh and observed that the facility sketch showed that there were three residents allowed to reside in two of the bedrooms located on the second floor. According to the fire clearance granted on 12/2024 the applicant, Rosa Dinh, was approved to be able to accept and allow for 3 residents in those (2) second floor bedrooms. It has been determined that the fire clearance will be under review after further consulting with upper management at this time. Additional communication will need to be held with the fire department as well.
Based on the information and review of the facility property, it has been determined that additional forms and documents will be required in order to further process this application.
This LPA will be in touch with the facility Applicant to schedule a follow up date for the Pre licensing visit.
An exit interview was held and a copy of the report was provided to Rosa Dinh.
