Facility Evaluation Report
On 05/19/2025 at 3:00 pm Licensing Program Analyst (LPA) Renee Campbell arrived at Espinoza Care Home #3 for the purpose of conducting a required 1 year annual inspection. LPA Campbell met with Michael Espinoza, Administrator and together they conducted a tour of the home.
LPA Campbell observed no residents in the facility. Per the administrator, the program design is still under review by Alta Regional California Center, the vendor for the facility. Once Alta approves the program design, the facility will be able to accept residents.
LPA Campbell and the Administrator evaluated the physical plant to ensure the health and safety of future residents in care. Areas inspected are including but not limited to the kitchen, resident bedrooms; resident bathrooms, living and dining room and outdoor areas. Two storage sheds were opened and observed by LPA Campbell to contain yard equipment and bags of clothing. LPA Campbell observed the facility to be free of odor, clean and in good repair and advised the administrator that a chair and lamp should be added to the resident's rooms. A bed, nightstand, linens, blankets and mattress cover were observed for the use of residents. This facility has not been utilized for client care in two years since obtaining licensure in 06/09/2023.
There are no staff or resident files to review.
Per California Code of Regulations, Title 22 there were no deficiencies cited during today's inspection. An exit interview was conducted, and a copy of this report was left at the facility.
