Complaint Investigation Report
On 07/07/2026, LPA Valerio conducted an unannounced visit. LPA Valerio inspected the hand rails around the interior of the facility. LPA Valerio observed many handrails to have chunks of wood missing from the handrail. One handrail was observed to have a piece of the hand rail missing, which exposed two needles or staples.
Based on interview and observation, the preponderance of evidence standard has been met, therefore the above allegations are found to be SUBSTANTIATED. California Code of Regulations (Title 22, Division 6, Chapter 8) are being cited on the attached LIC-9099D. Failure to correct the deficiency may result in civil penalties. Appeal rights were provided. An exit interview was conducted, and a copy of the report was left at the facility.
