Facility Evaluation Report
On 8/1/25 at 1:30pm, Licensing Program Analyst (LPA) Kevin Gould conducted an unannounced case management inspection to address the status of the facility residents relocations. LPA met with facility representative Carlo Esquillo and explained the purpose of the inspection.
LPA and facility representatives discussed the recent steps facility has taken to ensure the relocation of all residents following the death of the licensee. LPA has documented all relocation statuses and new placement addressed on LIC 811 and 811-C.
LPA observed there are still 5 residents in placement at this time. All residents have a new placement identified or been transferred to the hospital and awaiting placement per placement agency. There are currently 5 resident currently at the facility awaiting to be picked up today. LPA spoke with all residents present and all verbalized a relocation plan and plan to be transported to new placement today.
LPA Gould conducted a walk through of the facility and did not observed any deficiencies. LPA observed all residents rooms and belongings packed up to be transported to new placement.
Per the California Code of Regulations, Title 22 there were no deficiencies observed or cited during today's inspection. Exit interview conducted and a copy of this report and appeal rights were left at the facility.
