The inspector’s account

On August 6, 2026 at 2:00pm, Licensing Program Analyst (LPA) Sulma Lopez and Licensing Program Manager (LPM) Arielle Pascua arrived unannounced at the facility to conduct an annual inspection. LPA and LPM met with Facility Administrator Rustico Agcaoili and explained the purpose of the visit. The facility is approved for 3 ambulatory clients and 1 non-ambulatory client ages 18 to 59. The current census was 3 clients.

During the visit, LPA reviewed (4) staff and (3) client files. Staff files and client files are well maintained and current. LPA reviewed the facility's current Emergency Disaster Plan and Infection Control Plans. The facility is conducting monthly fire drills and the last drill was on July 30, 2026.

At 3:00PM, LPA and LPM toured the facility with the Administrator. LPAs observed the laundry room contained detergent locked in cabinets. The client bedrooms contained furniture in good repair. The facility was clean and free of odors. The restrooms were sanitary and contained non-slip mats. The facility has a walk-in linen closet with extra bedding and towels for client use. LPA and LPM toured the garage which was free of hazards. LPA observed the kitchen and dining area and there was a 7-day amount of non-perishable food items. The fridge contained at least 2-days worth of perishable food items. Centrally stored medications, cleaning supplies, and sharps were located in locked cabinets throughout the kitchen. LPA toured the backyard and observed that there was a covered patio with enough seating to accommodate the census. The emergency exit gate and fence were in good working condition.

During the visit, no deficiencies were cited. An exit interview was conducted and a copy of this report was provided to the facility.
