Facility Evaluation Report
On May 01, 2026, at 11:30 AM, Licensing Program Analyst (LPA) Avelina Martinez made an unannounced visit to this facility to conduct an annual required inspection. LPA Martinez met with Kassia Mitchell and explained the purpose of today's visit.
LPA Martinez inspected the physical plant including but not limited to the kitchen, dining room, resident bedrooms; resident bathrooms, laundry room, activity room, and outside courtyards of the facility to ensure compliance with Title 22 regulations.
Administrator holds current certificate. The facility is licensed for six ambulatory clients. There are currently four clients who reside at this facility.
The LPA Martinez toured the facility with on May 01, 2026 with Kassia Mitchell.
The facility has an infection control plan and a natural disaster plan. The last fire drill was conducted on April 09, 2026. LPA Martinez reviewed two staff files and two client files, and all records were maintained and kept current. LPA Martinez reviewed one medication administration record (MAR), and the MAR was maintained and kept current. LPA Martinez reviewed one PNI file, which was maintained and kept current. the facility has a first aid kit, and medication are stored in a locked cabinet. The facility smoke detectors, carbon detectors, and fire extinguisher were all in good repair. The emergency exit gate was in good repair. The facility had an adequate food supply. The facility temperature measured at 76 degrees, and the water temperature measured at 107 degrees.
Based on this annual inspection, there were no deficiencies cited. An exit interview was conducted, and a copy of this report was provided to the facility.
