Facility Evaluation Report

Licensing Program Analyst (LPA) Bethany Mirlohi arrived unannounced to conduct a case management visit. LPA spoke to Administrator, Simranjit Bhatia , during today’s inspection.

LPA investigated a complaint investigation and during the investigation LPA found deficiencies. LPA found that R1 was a diabetic and insulin dependent. Administrator informed LPA that caregivers (non-medical professionals) were injecting R1 with prescribed insulin injections. In addition, LPA found R1 had been sent to the hospital on several occasions and administrator failed to send incident reports into CCL.
