Facility Evaluation Report
On May 24, 2022, Licensing Program Analyst (LPA) DeAnna Williams-Lyons arrived unannounced to conduct an 1 year required annual Inspection. LPA met with Congmeng Lee, Program Director and explained the reason for the visit.
Prior to the visit, LPA completed required COVID-19 testing protocols, and a daily self-screening questionnaire for symptoms of COVID-19 infection to affirm no COVID-19 related symptoms; contacted licensee and completed a facility risk assessment. LPA ensured they applied hand sanitizer before entering the facility and the following Personal Protective Equipment (PPE) was worn: N-95 Masks.
Congmeng and LPA conducted the Covid infectious Control questionnaire with no advisories.
LPA and Congmeng toured the facility. The census is 7 at this time . The facilities Emergency Disaster Plan, Resident’s Rights and Facility Sketch was available for viewing. The room temperature was 74 degrees F which is within range. LPA observed there to be a sufficient amount of 2-day perishable and 7-day non-perishable food. Resident bedrooms and bathrooms were toured. There are 15 single bedrooms single suites. All rooms had the required items of furniture. Bathrooms were clean, sanitary and odorless and consisted of grab bars and non-skid mats. First Aid Kit was complete.
