Facility Evaluation Report

Licensing Program Analyst (LPA) Hiratsuka arrived unannounced at the facility to conduct a required annual visit. LPA met with Administrator and explained the purpose of the visit.

Today's visit, LPA and Administrator conducted a tour of the facility to ensure the health and safety of clients in care. Areas toured included but not limited to: four clients room, bathroom, storage room, kitchen, laundry room, dining room and the common areas. LPA observed clients doing activities in the dining room. LPA observed facility to have ample supply of perishable and nonperishable foods. LPA observed facility to have adequate supply of linens for clients in care. LPA did not observed any health, safety or personal rights violations.

One staff and one resident files were reviewed.

Exit interview conducted and a copy of report was provided.
