Facility Evaluation Report

On July 25, 2025, Licensing Program Analyst (LPA) Cassie Yang arrived unannounced at the facility to conduct a required annual inspection utilizing the care tool. LPA met with Licensee and explained the purpose of the visit.

LPA was informed Administrator is no longer working at the facility. LPA will was informed Licensee submitted documents to appoint Gerald Goode as Administrator. LPA informed Licensee that capacity has been changed from eight to four. New license was printed for Licensee.

Today's visit, LPA and Licensee conducted a tour of the facility to ensure the health and safety of clients in care. Areas toured included but not limited to: four clients room, bathroom, storage room, kitchen, laundry room, dining room and the common areas. LPA observed facility to be a comfortable temperature of 73*F. LPA observed clients doing activities in the dining room. LPA observed facility to have ample supply of perishable and nonperishable foods. LPA observed fire extinguisher recently serviced on March 25, 2025. LPA observed facility to have adequate supply of linens for clients in care. LPA did not observed any health, safety or personal rights violations. File review was conducted for two clients in care.

LPA and Licensee discussed age waivers for clients in care. LPA provided Licensee a copy of California Code Regulation 85068.4 Acceptance and Retention Limitations.

Care tool completed and found facility to be in compliance.

Exit interview conducted and a copy of report was provided.
