Facility Evaluation Report
On November 18, 2025, Licensing Program Analyst (LPA) Cassie Yang arrived unannounced at the facility to conduct a case management visit regarding a death report LPA received for client (C1) on October 31, 2025. LPA met with Administrator and explained the purpose of the visit.
Today's visit, LPA obtained a copy of C1's LIC 602 Physician's Report For Community Care Facilities and C1's clinical notes.Administrator informed LPA once death certificate is generated and issued. A copy will be provided to LPA.
At this time, the incident remains under review by the Department until further notice.
Exit interview and a copy of the report was provided.
