Facility Evaluation Report

On July 29, 2025, at approximately 3:30 PM, a virtual office meeting was held between the Licensing Program Analyst (LPA) Cheyenne Ratajczak, Licensing Program Manager(LPM), Troy Ordonez and the Licensee of the facility, Ellen Huffhines and Administrator Jerry Huffhines. The meeting was scheduled to address documentation updates required following the passing of the co-licensee, who had also been listed as the administrator.

During the meeting, CCL reviewed the facility file and advised the licensee that the following documents were either outdated or missing. The licensee was informed that to maintain compliance with licensing regulations, the following updated forms must be submitted to LPA, Cassie Yang by August 15, 2025:

· LIC 308 – Designation of Responsibility

In addition, CCL is requesting the following supportive documentation to be submitted to Licensing Program Analyst, Cassie Yang by August 29, 2025:

· Current Administrator Certificate or proof of registration in a 35-hour ARF Administrator Certification Program.

· Proof of control of the property was submitted to LPA Cassie Yang.

Exit interview and a copy of this report will be provided to the facility via email. A copy will be signed and returned to CCLD.
