Facility Evaluation Report
On October 23, 2025, Licensing Program Analyst (LPA) Cassie Yang arrived unannounced at the facility to conduct a required annual inspection utilizing the full CARE tool. LPA met with Administrator, and explained the purpose of the visit.
LPA observed two clients at the facility while two other clients were at adult day program.
Today's inspection, LPA and Administrator conducted a tour of the interior and exterior of the facility. Areas toured included but not limited to: clients bedrooms, bathroom, garage, kitchen, backyard and the common areas. LPA observed facility to be at a comfortable temperature for clients in care. LPA observed facility to have two days of perishable and seven days of non perishable foods. LPA observed thermometers in facility freezer and refrigerator to ensure it remains compliance. LPA observed clients bedrooms to be free of dangerous weapons, medication and toxins. LPA observed sharps to be locked and secured. LPA observed backyard to be free of dangerous weapons and no obstructions in pathway. Fire extinguisher observed to be recently serviced. In areas toured, no immediate health, safety, personal rights violation observed.
File review was conducted of four clients records and four personnel records. LPA observed client records to be complete. LPA observed personnel records to proof the training and criminal record clearance. File review of Emergency Disaster Plan revealed plan was recently reviewed. File review of fire drill records revealed fire drills are conducted monthly at the facility.
CARE tool completed and found facility to be in compliance. No deficiencies observed.
Exit interview and a copy of the report was provided.
