Facility Evaluation Report
Licensing Program Analyst (LPA) Cassie Yang arrived at the facility to conduct a required annual inspection utilizing the full CARE tool. LPA met with Administrator and explained the purpose of the visit.
Today's inspection, LPA and Administrator conducted a tour of the facility to ensure the health and safety of clients in care. Areas toured included but not limited to: clients bedrooms, staff room, office, bathrooms, kitchen, backyard, garage and the common areas. LPA observed clients room to be no more than two clients per room. LPA observed facility to be at a comfortable temperature of 73*F. LPA observed kitchen sharps to be locked and secured. LPA observed facility to have at least two days of perishable and seven days of non-perishable foods. LPA observed medications and toxins to be locked and secured. Toxins were observed to be stored separately from food supplies. In areas toured, no deficiencies observed. LPA observed client bathroom to be clean and free of malodor.
LPA conducted a file review for three clients records and staff records. Staff records reviewed indicated current first aid a nd cardiopulmonary resuscitation , alon g with Direct Support Professional Training completion.
LPA observed monthly fire drills conducted at the facility for year of 2025. Facility is compliance to Title 22 of quarterly emergency disaster drills. Emergency Disaster Plan was recently reviewed on August 1, 2025 by Administrator.
Administrator reported no concerns at the facility at this time. CARE tool was completed and found facility to be in compliance. No deficiencies cited.
