Facility Evaluation Report
Licensing Program Analyst (LPA) Christina Valerio arrived unannounced to conduct an annual required inspection. LPA Valerio met with facility staff Emmirrie and was later met by Administrator Divina Joseph.
LPA Valerio and facility staff toured the facility to ensure compliance with Title 22 regulations. LPA Valerio observed resident bedrooms to be clean, fully furnished, and personalized to each resident's interest. LPA Valerio observed bathrooms to be sanitary. The facility has a closet stocked with hygiene supplies and resident's hygiene basket. The facility was observed to have toxins, sharps, and medications locked and inaccessible to residents in care. Staff room was observed to be utilized for staff only and facility files. The facility was observed to have a first aid kit and an emergency supply of food and water. The fire extinguisher was observed to be fully charged and operational. The last emergency disaster/fire drill was conducted the first week of February.
LPA Valerio was sent an Unusual Incident Report via email regarding an incident that occurred on 01/12/2026. It was reported that during Alta California Regional Center's unannounced visit, they found a documentation error for two PRN medications and missing information on a PRN Authorization letter. The facility was required to complete medication training from ARCA-Learn. Technical Advisory was discussed with Divina regarding the incident and reporting requirements.
LPA Valerio reviewed one staff file and one resident file. All files were observed to be up to date with required information. LPA Valerio requested the following be sent via email: LIC 500, LIC 308, LIC 610D, and a copy of their surety bond.
Per California Code of Regulations (CCR) - Title 22 - no deficiencies were observed. An exit interview was conducted with Administrator Divina, and a copy of this report was provided.
