Facility Evaluation Report
Licensing Program Analyst (LPA) Christina Valerio arrived to the facility unannounced to conduct an annual required inspection. LPA Valerio met with facility staff Albert, and explained the purpose of the visit. Facility staff mentioned that Administrator Susie is out of town.
Upon arrival, LPA Valerio observed resident sitting under the shade smoking a cigarette. Resident stated they are enjoying the warm weather and would go inside shortly. A few minutes later, staff was observed assisting resident back inside. The facility is a two story home. The top floor is designated for staff and the bottom floor is for residents. LPA Valerio and facility staff conducted a tour of the facility. LPA Valerio observed resident bedrooms to be fully furnished and free from odors. Room 1 had a television that did not have working sound. According to staff, a technician will be coming out to fix the sound on Monday. Resident bathroom was observed to be fully stocked with hygiene supplies, hand rails, trash can, and skid matt. The hot water was observed to deliver hot water at 110.8 degrees Fahrenheit, which is within the required regulatory range. The facility was observed to be have adequate food supply, in addition to an emergency supply of food. The fire extinguisher was fuller charged and last service on 02/27/2026. LPA Valerio did not observe any hazardous items accessible to residents in care.
LPA Valerio observed two resident files and two staff files. LPA Valerio requested the following be sent to christina.valerio@dss.ca.gov: LIC 500, LIC 308, LIC 610D, copy of liability insurance, and copy of administrator certificate.
Per California Code of Regulation (CCR) - Title 22 - no deficiencies were cited. An exit interview was conducted with staff, and a copy of this report was provided.
