Facility Evaluation Report

Licensing Program Analysts (LPA's ) Christina Valerio and Kimberly Viarella conducted a case management visit to the facility for the purpose of delivering an Order to Licensee/Facility of Immediate Exclusion From Facility for all facilities for Staff 1 (S1), Staff 2 (S2), and Staff 3 (S3).

LPA's met with Administrator Susie Dizon, and explained the purpose of visit. S1, S2, and S3 is excluded as a result of actions not related to this facility. S1, S2, and S3 were not observed at the facility during this time.

LPA's handed the Order to Licensee/Facility of Immediate Exclusion From Facility and explained that S1, S2, and S3 cannot come to the facility and cannot be allowed to work, be present and/or live in a CCL licensed facility or have contact with clients in any residential facility or child day care licensed by the California Department of Social Services effective January 28, 2026. Licensee was informed to remove S1, S2, and S3 from their guardian roster as soon as possible.

Per California Code of Regulations (CCR) - Title 22, no deficiencies were observed. An exit interview conducted, a copy of this report provided on this date. A signature on these forms acknowledges receipt of these forms.
