Facility Evaluation Report
On July 3, 2026, Licensing Program Analyst, Arvin Villanueva (LPA) arrived at this facility unannounced to conduct the annual inspection visit. LPA initially met with staff on duty and stated the purpose of the visit. The Administrator, Raehan Astronomo (AD), was notified and arrived shortly after. Present during this visit were 3 residents in care with 2 staff on duty.
Overview : Facility is a one-story house located in a residential neighborhood. Facility is licensed to serve up to 4 adult residents, up to 4 may be non-ambulatory. Facility does not have clearance for bedridden, delayed egress, and/or locked exterior and/or interior.
Physical Inspection : Areas inspected include, but not limited to, the kitchen, dining, resident units/bedrooms, resident bathrooms, common areas and outdoor areas.
LPA inspected 4 of 4 bedrooms and two bathrooms. Hot water temperature was measured at 117 degrees Fahrenheit. Hallway temperature was maintained at 71 degrees Fahrenheit.
Fire extinguisher was observed in the kitchen and was last inspected on 2/23/26. There is a fire door leading to bedrooms 3 of the resident bedrooms. Fire door was observed to be closed throughout this visit. Smoke and carbon monoxide detectors were observed throughout. Cleaning solutions and other toxic substances were observed to be locked and not accessible.
