Facility Evaluation Report
Licensing Program Analyst (LPA) Christina Valerio arrived unannounced to conduct a pre-licensing visit. LPA Valerio met with facility staff Carmen Ragas, and explained the purpose of the visit. Facility staff contacted administrator Flordeliza Navarro via cell phone and arrived shortly after.
LPA Valerio toured the facility to ensure compliance with Title 22 regulations. LPA Valerio observed resident bedrooms to be fully furnished, free from odors, and personalized to each individual interest. Resident bathrooms were observed to be clean and sanitary. Common areas (living rooms, dinning room, hallways, and kitchen area) was observed to be organized, free from debris, and fully furnished. Emergency exits were clear from obstructions. Medications, sharps, and toxins were observed to be locked and inaccessible to residents in care. The exterior area was observed to have an area for sitting, gardening, and outdoor activities. Fence panels were in good repair and no obstructions to emergency exits were observed. LPA Valerio observed the fire extinguisher to be fully charge, carbon monoxide detectors/fire detectors to be in working condition, and a fully stocked first aid kit. Staff was observed assisting resident with medications, snacks, and going to an appointment.
LPA Valerio reviewed staff and resident files. All files were observed to be up to date with required documentation.
LPA Valerio requested the following be sent via email to cclascpsacramentosouthro@dss.ca.gov: LIC 500, LIC 308, LIC 610, and copy of surety bond.
No deficiencies were observed today. An exit interview was held, and a copy of this report was provided.
