Facility Evaluation Report
On 04/02/25, Licensing Program Analyst (LPA) Kimberly Viarella made an announced visit to this facility to conduct a prelicensing inspection. The LPA identified herself upon arrival, stated the purpose of the visit and asked to meet with the facility Administrator/Licensee. LPA met with Eveleth Purugganan and a brief interview followed.
There were no residents in care as this was a new facility. LPA and Administrator/Licensee toured the facility together. All sharps and toxic chemicals were locked and inaccessible to future residents in care. Hot water measured 112 degrees Fahrenheit and in compliance at the time of this inspection. LPA inspected all 4 resident rooms and each had the required furniture, furnishings and lighting to be in compliance at the time of this inspection. This facility also had 1 designated staff room which the LPA viewed during this inspection. LPA verified that the First Aid Kit had all of the required elements and was in compliance. The Administrator/Licensee also showed the LPA designated locked cabinets for medications, PRNs and cleaning supplies. The facility was also equipped with games for residents to enjoy.
LPA observed the following materials posted on the wall in the entry way: The Complaint/Grievance Procedure with form, If You See Something Say Something sign, Ombudsman sign, Resident Schedule of activities, Facility sketch, and the Emergency / Disaster Plan.
LPA inspected the exterior of the building. All window screens and gutters were in good repair and there were not bodies of water present. There was a shaded area with furniture for the future residents to enjoy.
According to the California Code of Regulations, Title 22, there were no deficiencies observed during today's visit. This facility passed both its inspection as well as COMP III and is ready to proceed with licensing.
