Facility Evaluation Report
Licensing Program Analyst (LPA) Christina Valerio arrived unannounced to conduct a case management visit to follow up on the Technical Advisories from the annual visit conducted on 02/10/2026. LPA Valerio was met by facility staff, and explained the purpose of the visit. LPA Valerio was later met by Administrator Simranjit Bhatia.
LPA Valerio toured the facility to ensure compliance with Title 22 regulations. LPA Valerio observed exit and fire doorways clear and free from obstructions. LPA Valerio observed five (5) residents in care. LPA Valerio observed home health attending to resident in care. LPA Valerio inpsected the shed in the back to be utilized for storage.LPA Valerio discussed incident reports submitted in the last few months. According to Administrator Simranjit, there had been one recent fall, which did not result in any injuries. LPA Valerio and Administrator discussed the option to refer facility to the Technical Assistance Program (TSP). TSP was discussed offered during the 02/10/2026 visit and via e-mail. Administrator stated she would like the regional office to refer her to TSP.
LPA Valerio reviewed facility files, which were out of compliance of Title 22 regulations. Resident file reviewed did not have a centrally stored medication log. Staff files reviewed did not have a copy of annual training, such as First Aid and Dementia Care.
Per California Code of Regulations (CCR), Title 22, Chapter 8, deficiencies are being cited and can be found on the attached LIC 809 - D page. Appeal Rights provided. An exit interview was held, and a copy of this report was provided.
