Facility Evaluation Report
Licensing Program Analyst (LPA) Christina Valerio arrived to the facility unannounced to conduct an annual required inspection. LPA Valerio was met by facility staff Melkeet, and explained the purpose of today's visit. Facility staff contacted the administrator who stated she would come to meet LPA.
LPA Valerio and facility staff toured the facility to ensure compliance with Title 22 regulations. LPA Valerio observed resident bedrooms. Resident bedrooms were observed to free from odors and fully furnished. The bathrooms were observed to stocked with hygiene supplies, a skid mat, a garbage can, and handrails. Common areas were observed to be fully furnished and organized. The thermostat inside the home was set at a comfortable temperature of 72 degree F. The kitchen was observed to have working appliances and an adequate supply of food. The fire extinguisher located in the kitchen was observed to have an annual inspection date of October 15, 2024. Administrator was able to provide LPA proof that they had their service for the fire extinguisher on today's date. LPA was provided a copy of the service with an updated service tag.
LPA Valerio observed three resident files and two staff files. LPA Valerio provided Technical Assistance. LPA Valerio provided guidance on resident annual documentation for residents, appraisals, and exception process. LPA Valerio discussed the Technical Support Program and will send information to Administrator via email.
LPA Valerio requested the following documentation be sent to CCLASCPSacramentoSouthRO@dss.ca.gov: LIC 500, Facility Sketch, LIC 308, LIC 610, and Copy of Liability Insurance.
An exit interview was held, and a copy of this report was provided.
