Facility Evaluation Report

A Non-Compliance Conference (NCC) was conducted today on May 18,2026, via Microsoft Teams with

the Sacramento South Regional Office. The purpose of this NCC meeting was to discuss deficiencies and that have occurred within the last 12 months. Present in the meeting is Regional Manager (RM) Stephenie Doub, Licensing Program Manager (LPM) Stephen Richard, Licensing Program Analyst (LPA) Christina Valerio, and LPA Reza Jamaly; Facility representatives include Licensee Tawanda Meda, Facility Staff Janet Meda, and Administrator Phillip Omole; Alta California Regional Center Representatives include Quality Assurance & Compliance Specialist Wayson Yuen, Quality Assurance & Compliance Manager Scott Barr, Quality Assurance Director John Barr, Community Services Specialist Jolanna Araujo, and Community Services & Supports Manager Michelle Duchene.

During this virtual meeting, the Non-Compliance Conference process was explained to the Licensee. A

Non-Compliance Conference Summary (LIC 9111) was generated to document this office meeting. A copy of this report and LIC 9111 was provided to the licensee via e-mail. The facility has previously received one (1) Type A Violation and has been licensed since March 06, 2025.
