Facility Evaluation Report
On 10/07/24, Licensing Program Analyst (LPA) Kimberly Viarella made an unannounced visit to this facility to conduct an annual inspection. The LPA identified herself upon arrival, stated the purpose of the visit and asked to speak to the designated facility Administrator, Gina Wasdyke. Staff contacted the Administrator and she arrived after approximately 20 minutes. A brief interview followed.
LPA began the visit by comparing the Guardian Roster to the LIC 500. All staff were cleared and the facility was in compliance at the time of inspection.
LPA inspected all common areas including by not limited to the: kitchen, dining room, living room. These areas contained the required furniture, furnishings, and lighting to be in compliance.
This LPA went on to inspect the 4 resident bedrooms. These too, contained the necessary items to be in compliance at the present time.
The fire extinguishers were last inspected on 11/01/23 by Jorgensen Co. and were in compliance at the time of inspection.
LPA tested the hot water in bathroom 1 located closest to the kitchen. The water temperature registered 108.8 degrees Fahrenheit and was also in compliance. LPA observed that both bathrooms had grab bars, soap, and paper towels and were in compliance.
LPA observed the Administrator Certificate for Gina Wasdyke (#7033028735) was posted with an expiration date of 12/11/25. The Designee for this facility, Angeline Torres, RN, also had her Administrator's Certificate (#7011084735) posted with an expiration date of 01/06/26.
