Staff did not ensure that the client’s therapeutic care needs were properly met at the facility.

On 3/16/26, Licensing Program Analyst (LPA) Talwinder Bains arrived at the facility unannounced to deliver complaint findings for above allegation. LPA met with staff and explained the purpose of the visit. Regional Director of Operations, Vanessa Potier Watts came after short while and assisted LPA with today's visit.

During investigation, the Department interviewed four (4) residents and four (4) staff and reviewed records to investigate this allegation. Record review indicated that facility kept proper documentation regarding residents’ admission agreements, medical assessments, needs and service plan, medications records, staff’s care notes and other required components regarding resident’s care per Department’s Regulations. Residents’ interviews reflected that facility was meeting their care needs per their needs and service plan and per admission agreements. Resident’s interviews provided satisfaction regarding therapy services at facility however residents have their own choices and schedule regarding facility’s outings and other therapeutic services. Staff interviews indicated that they were aware of residents’ care needs per their needs and service plans and providing care accordingly without any issues. Staff interviews reflected that facility offer facility outings and other therapeutic services for residents however these plans can change per resident’s needs, staffing and weather conditions. Based on gathered information, this allegation was found to be UNSUBSTANIATED.
