Facility Evaluation Report

On 11/25/25, Licensing Program Analyst (LPA) Talwinder Bains arrived unannounced to conduct a case management inspection following receipt of incident report (IR/ LIC624) for a resident ,R1 on 11/14/25. LPA met with Program Manager, David Rosario and explained purpose of inspection.

IR indicated that R1 was residing at the facility from 11/11/25 and R1 left facility by choice on 11/14/25 around 08:45 AM as R1 walked away from the facility with their belongings. Staff attempted to redirect R1 from leaving facility but was not successful. Staff notified law enforcement . A police report was filed case number 20-352780, Officer Badge # 222. LPA reviewed R1s LIC602, needs and service plan and other documents related to this incident. R1s LIC602 (dated-11/11/25) indicated that R1 can leave facility unassisted. Facility notified all required parties regarding this matter and took appropriate action to address this matter.

There are no deficiencies being cited during today's inspection per Title 22 Regulations. .

Exit interview was conducted. Copy of report left at facility.
