Facility Evaluation Report
On 09/16/25, Licensing Program Analyst (LPA) Talwinder Bains arrived unannounced to conduct a case management visit. LPA met with Regional Director of Operations, Vanessa Potier Watts and explained purpose of inspection.
On 06/17/2025, the Department conducted a case management visit to follow up on the death of R1. The department conducted an investigation which found that R1’s death was determined to be from natural causes.
Based on the information obtained, R1’s death was not due to negligent from the facility therefore no citations will be issued due to this incident. Exit interview was conducted and a copy of the report was provided.
