Facility Evaluation Report
Licensing Program Analyst (LPA) Talwinder Bains arrived at the facility and met with Regional Director of Operations, Vanessa Potier Watts to conduct an unannounced case management visit on 5/6/26.
LPA observed staff, S1 was present at the facility with presence of all residents. Department has followed up regarding staff, S1, Guardian Fingerprint clearance and eligibility to work in CCL facilities as indicated in case management visit with the facility on 3/16/26 and facility was aware the S1 cannot work or present at the facility until they were Fingerprint cleared and eligible to work.
LPA Bains notified Licensing Program Manager, LPM, Laura Munoz regarding this issue via phone. As of today, S1 was Not cleared to work per Guardian database.
Department discussed that staff, S1 was asked to comply with requirements to determine Exemption Eligibility to work in facility on/around July 2025 and certified mail was sent to facility and S1's home address but that case was closed since S1 did not respond to that letter so at this time this case has been closed in Guardian. Based on this information, staff member (S1) cannot be allowed to work, be present and/or live in a CCL licensed facility, or have contact with clients in any residential facility or child day care licensed by the California Department of Social Services until S1 comply with these requirements to determine their eligibility to work.
Based on gathered information, citation has been issued per Title 22 Regulations as listed on LIC809-D. Immediate civil penalty of $500.00 was issued today.
Exit interview was conducted and a copy of this report and appeal rights were provided.
