Facility Evaluation Report
Licensing Program Analyst (LPA) Talwinder Bains arrived at the facility unannounced on 11/18/25 to do case management visit . LPA met with staff, DSP, Matthew Wilson and explained the purpose of the visit. LPA and staff spoke with Program Administrator, Latisha Woodson (LW) via phone who gave permission to conduct today's visit with staff as LW was not able to come to the facility.
LPA conducted this visit as department was notified on 10/31/25 via letter by licensee that this facility will be closing down as facility is unable to financially sustain the staffing ratios, expenses, and overtime within the current rate structure and has identified the need for voluntary closure. The tentative date for facility closure is 04/30/26. This letter was sent to department to put 180 days closure notice per CCLD requirement.
During today's visit, LPA was informed by staff that facility already put the notifications to all residents, responsible parties, ALTA Regional Center and other required agencies regarding facility's closure. Per staff, facility is working with all parties to have a smooth transition regarding residents relocation to other facilities due to this matter. Facility will notify CCLD once all residents are moved out and/or for any other questions regarding this issue.
During today's visit, no health or safety concerns were identified.
No deficiencies were observed or cited from today's visit.
