Facility Evaluation Report
Licensing Program Analyst (LPA) Talwinder Bains arrived at the facility unannounced on 09/18/25 to do case management visit . LPA met with Assistant Administrator, Autiana Brown and explained the purpose of the visit.
Department followed up on SOC 341 and incident report sent by facility on 09/17/25 which involved resident, R1 and staff S1 for date-09/17/25 . Facility notified LTCO, responsible party and other agencies as required regarding this matter.
Department conducted interviews today and requested documents related to this matter.
At this time, this incident is under review and department will do follow up as needed.
No citations were issued per Title 22 Regulations from today's visit.
Exit interview conducted and copy of the report left at facility.
