Facility Evaluation Report

An office meeting was conducted at 10:30am on April 24, 2026, with Sacramento North Regional Office. The purpose of this meeting was to discuss the issuance of an EAO due to the passing of the licensee.

The following Licensing staff were present:

Licensing Program Managers (LPM) Laura Munoz, and Licensing Program Analyst (LPA) Lavinia Muscan

The following facility representatives were present:

Administrator Nicoleta Osanu and Applicant, William Osanu

The following documents were provided during today's meeting:

LIC200, LIC500, Administrator Certificate, Control of Property, Death Certificate and application fee.

The Department issued William Osanu an Emergency Approval to Operate (EAO) during today's meeting. The Applicant was advised that the documents provided will be forwarded to the Central Applications Bureau (CAB) for processing.

An exit interview was conducted and a copy of this report was provided.
