Facility Evaluation Report
Licensing Program Analyst (LPA) Talwinder Bains arrived at the facility unannounced on 4/17/26 to conduct a Case Management. LPA met with the staff and explained the purpose of today's visit. Staff , William Osanu came after short while and assisted LPA with today's visit.
During today's visit, the Department checked the food supply and did a brief walk through the facility with staff. LPA reviewed resident, R1s file and checked resident, R1s medications. No concerns noted. Several topics were discussed. No immediate health and safety concerns were noted during this visit.
No citations were issued per Title 22 Regulations from today's visit.
Exit interview conducted and copy of the report left at facility.
