Facility Evaluation Report
Licensing Program Analyst (LPA) Talwinder Bains arrived at the facility unannounced on 5/20/26 to conduct a Case Management. LPA met with Caregiver Fidel Ocampo and explained the purpose of today's visit. LPA spoke with Assistant Administrator, William Osanu regarding today's visit via phone.
During today's visit, the Department checked the food supply and did a brief walk through the facility with staff. LPA observed all six residents at the facility during this visit. No concerns noted. No immediate health and safety concerns were noted during this visit.
No citations were issued per Title 22 Regulations from today's visit.
Exit interview conducted and copy of the report left at facility.
