Licensing Program Analysts (LPAs) Lavinia Muscan and Talwinder Bains arrived unannounced to conduct an annual inspection. During the inspection it was determined that Administrator altered the expiration date of his certificate. Based on evidence found, this case management visit was conducted and deficiency was observed and cited.
The following deficiency (1) is cited per California Code of Regulations, Title 22. See 809-D for the citation issued. Exit interview done. Copy of report and Appeals Rights were provided.
