Licensing Program Analyst (LPA) Talwinder Bains arrived on 5/7/26 to conduct an unannounced case management visit. LPA met with Administrator, Ionel Dascalescu and explained the purpose of today's visit. This visit is a health and safety check in response to R1 who was relocated to this facility on 5/4/26 from another facility.
LPA spoke with R1 today. LPA toured R1's room and conducted facility's tour.
Exit interview was conducted and a copy of this report was provided to the facility.
