Licensing Program Analyst (LPA) Bethany Mirlohi arrived unannounced to conduct a case management visit. LPA met with Administrator Emanuel Dirar during today's inspection.
The department received an incident report concerning R1 sustaining an unwitnessed fall on June 17th. Resident was sent out to the hospital and returned back to the facility the same day. On June 18th resident continued to complain of pain and was sent out to the hospital again and was discharged to a skilled nursing facility with a hip fracture. PCP and family were notified.
At the time of this visit no deficiencies were given.
