On 7-8-2026 at 2:27pm, Licensing Program Analyst (LPA) Michael Bilger arrived unannounced to conduct a collateral visit regarding facility #392700109 and the associated complaint #27-AS-20260424151028. LPA met with Administrator Antonette Edwards and explained the purpose of the visit. The purpose of today's visit was to interview resident1 (R1) as part of the on-going investigation. Upon arriving at the facility, LPA learned that R1 is no longer residing at the facility. LPA received additional contact resources for R1 for purpose of continuing investigation.
An exit interview and a copy of this report was provided to Administrator.
