Licensing Program Analyst (LPA) Hiratsuka, conducted this unannounced case management visit in response to an incident that was reported April 24, 2026 and April 27, 2026.
The floor is having issues. There is water issue that is affecting the floor and is making the floor bubble in places. The facility is going to relocate temporarily to a hotel while the building is repaired.
LPA toured the facility today. The residents have all been notified. The responsible parties have been notified. No hazards were observed today.
