The inspector’s account

On September 30, 2025, Licensing Program Analyst (LPA) Cassie Yang arrived at the facility unannounced to conduct a required annual inspection. LPA met with Administrator and explained the purpose of the visit.

LPA and Staff conducted a tour of the interior and exterior of the facility to ensure the health and safety. Areas toured observed the program office, dining room, men shower room, men bathroom, women shower room, women bathroom, clinical rooms, medication room, television room, resident bedrooms, and the common areas. LPA observed facility to have the mandated posters posted in the common area. LPA observed fire extinguishers present throughout the facility, extinguishers observed to be recently inspected. LPA observed bathrooms to have grab bars for residents usage. LPA observed telephone available in the hallway for residents in care. In areas toured LPA observed the facility to be clean, sanitary and in good repair. LPA did not observe any concerns during facility inspection.

LPA and Administrator discussed the medication pass policy to ensure resident are assisted as needed with self administered medications. Additionally, LPA and Administrator discussed the concerns at the facility, Administrator stated she will be monitoring and reassessing residents in care to ensure needs are being met.

At this time, LPA is requesting a copy of liability insurance and medication pass policy.
