Facility Evaluation Report
On February 7, 2025, Licensing Program Analyst (LPA) Cassie Yang arrived unannounced at the facility to conduct a case management visit regarding an incident report the Department received. LPA met with Administrator and explained the purpose of the visit.
The incident occurred on January 18, 2025, when night shift medication technician had marked a medication as given but it was later observed the medication was not administered. Administrator explained that S1 was in the middle of doing a medication pass when S1 was interrupted by another resident in care for assistance.
Administrator stated corrective actions were taken and S1 will be retraining on medication administration through Relias.
Administrator will submit S1's medication training to LPA Yang via email by February 14, 2025.
